关于申领上海考点2019年第二次考试（含）以前尚未领取的CIA/CCSA证书的公告：

于2019年第二次考试（于2019年6月举行）或更早时间全科通过考试且尚未领取证书的上海考点考生（即当时CCMS中，“个人信息”中“最能描述您地理位置的分会/协会”选项为“IIA（中国）724–上海”），从即日起可按以下步骤领取中、英文证书：

一、发送证书申领邮件

请发邮件至certification@ciia.com.cn，邮件标题：全科通过考试时间（年/月），CIA/CCSA证书领取（上海），姓名（中文），考生ID号，认证ID号。

请在邮件正文中提供以下信息：（1）申领人姓名（中文）和CCMS中显示的英文（拼音）姓名；（2）申领人的邮寄地址及手机号。

同时以附件形式提供申领人本人身份证件正反面的扫描件或图片。

二、邮寄申领人本人照片

邮寄一张申领人本人近期的两寸彩色照片（底色无要求）至北京市海淀区中关村南大街1号北京友谊宾馆嘉宾楼一层 中国内部审计协会国际事务部，电话010-82199868。为避免丢失，推荐使用顺丰快递。

邮寄前请在照片背面注明申领人姓名及考生ID号，并在快递封套上写明以下信息：“全科通过考试时间（年/月），CIA/CCSA证书领取(上海)，姓名，考生ID号，认证ID号”

注：

1.全科通过考试时间可在新版CCMS中查看，即CCMS主页上“我的CIA认证”下，显示的“授予日期”。

2.考生ID号为纸笔考试时的准考证号，即原CCMS主页持证人员姓名后的7位数字。新版系统中位于主页个人信息中持证人员姓名后的括号内。

3.认证ID号即为证书号，新版CCMS中主页上，“我的CIA认证”下显示的“认证ID#”。

中国内部审计协会在收到证书申领邮件后会回复告知证书制作程序，待收到照片后，将开始证书制作，并在确认邮寄信息后，以快递方式寄达申领人，邮寄费用由申领人承担。
